Our Privacy Policy Statement
My Dental is committed to uphold and protect the privacy of patients and to maintain patients’ confidential information in accordance with the Personal Information Protection Act (PIPA), the Health Information Act (HIA), and the Alberta Dental Association and College Code of Ethics. My Dental will not collect, use, or disclose any personal information that is not in line with PIPA, the HIA, or the policies set forth by My Dental.
Principles
My Dental employees will maintain the strictest confidence of personal information collected from patients.
Information collected will be used solely for providing dental services and associated business services.
My Dental employees are trained in and must comply with applicable policies to uphold the privacy of patients and the confidentiality of personal and health information collected, used, and stored.
Consent
By booking an appointment with My Dental, you consent to the use of your name and contact information for the purpose of scheduling appointments. By signing the My Dental Personal Information Consent Form, you consent to the collection and use of your personal information as described in the form and this Privacy Policy. Your personal information will not be collected, used, or disclosed for any other purpose without your consent.
Types of information My Dental collects
My Dental collects personal information including your name, email address, home address, phone number, dental history, financial payment information, and medical information necessary to provide dental services. Only that information which relates directly to and is necessary for the delivery of our services is collected as provided by you.
Use of Personal Information
Collected personal information will only be used by authorized My Dental employees to fulfill the purpose for which it was originally collected, for specific purposes if directed by the individual from whom the personal information was collected, or for a use consistent with the original purpose.
Personal Information Disclosures
My Dental treats personal information in our custody and/or control with the highest degree of confidentiality and will only disclose it for the purpose for which it was collected, or as required by law, or in specific circumstances with consent of the relevant individual.
Records Retention and Disposal
Patient records are kept indefinitely. When an individual is no longer a patient of My Dental, their account is deactivated but may be reactivated upon request.
Access to Information
You are entitled to access all personal information that we have in our custody and control or to make changes to your personal information. You may access or change your own personal information by submitting a request in writing to: omar@Mydentalgroup.ca
Security
My Dental protects your personal information by ensuring reasonable security arrangements against such risks as unauthorized access, collection, use, disclosure, or disposal are in place. Security measures have been integrated into the implementation, and day-to-day operating practices as part of My Dental’s commitment to the protection of the personal information we hold.
Breach of Privacy
If the event of a privacy breach, My Dental will operationalize their Privacy Breach Protocol.
Privacy Impact Assessments
My Dental Privacy Impact Assessment(s) will be reviewed on an annual basis or as required to ensure continued compliance with the Health Information Act, Personal Information Protection Act and other related legislation. Additional Privacy Impact Assessments will be created as required.
